Learning Disabilities & Mental Health
At Twenty Four Seven Nursing we recognise that many people with learning disabilities or mental health needs live active, independent lives, however, we also know that for many others, obtaining the right support can be the difference between reaching ones full potential and living a life of challenge and isolation.
We provide support to individuals both privately and to those referred by Social Services, or who are purchasing services through Direct Payments or Individual Budgets.
We currently provide support to a range of clients including those with:
- Mild, moderate and severe disabilities
- Complex needs
- Autistic Spectrum Disorder
- Down Syndrome
- Challenging Behaviour
- Severe Hearing Impairment
- Compounding Mental Health needs
We work in partnership with the people who use our services, their families, local authority and PCT commissioners, GPs, clinicians and carers to ensure effective health and social care solutions that benefit everyone are reached.
Consistent with our ‘Person Centred’ philosophy of care our aim is to empower clients to make their own decisions and live as independently as possible. We offer support with personal care, as well as help and support for each person to learn new skills, develop independence, access other services and enjoy life as part of the community. Each person has detailed, outcome focussed Support Plan and an Activity planner, information includes personal care, learning opportunities, recreational, social, vocational and work activities. It may also include support to access community facilities such as college, work or leisure.
